Consultants Laboratory of WI, LLC is the provider for the Wellness Lab Screening. All individuals obtaining a blood test of any kind will need to pay $3.00 for the blood draw. The price list below does NOT include the $3.00 blood draw charge.

To have results sent to your home, please bring a self-addressed stamped envelope or come to the Wellness Clinic Thursday, October 20th from 9:00a.m.-10:00a.m., 11:30am-12:30pm, or 2:00pm-3:00pm for an explanation of you test results. All critical labs will be called to your M.D.
