Offered by Lutheran Social Services, Outagamie County Aging and Disability Resources Center and Consultants Laboratory of WI, LLC is the provider for the Wellness Lab Screening. All individuals obtaining a blood test of any kind will need to pay $3.00 for the blood draw. The price list below does NOT include the $3.00 blood draw charge.

To have results sent to your home, please bring a self-addressed stamped envelope or come to door B at the Richmond Street Office, Thursday April 20th from 9:00a.m.-10:00a.m., 11:30am-12:30pm, for an explanation of you test results. All critical labs will be called to your M.D.
