Fox Valley Hematology & Oncology (FVHO), in partnership with Heartland Hospice Care, will host a workshop where the public can come at their convenience and receive assistance in completing their healthcare power of attorney and medical living wills.
Power of attorney gives another person the authority to act on someone else’s behalf in legal or health matters, while a medical living will is legal, documented instructions regarding a person’s preferences for medical care if they are unable to make those decisions. These forms typically take 20 to 30 minutes to complete.
FVHO physician Dr. Jack Swanson will be available during that time frame to assist and answer questions.
