ACP & ME – Getting Started: Advance Care Planning, will be held on Thursday, April 20 from 4-5:30 p.m. at the Hoffman Memory Care Resource Center at St. Paul Elder Services in Kaukauna. The entrance is located at 221 E 13th Street (map). This free seminar will help raise awareness about the importance of advance directives, provide resources on how to complete them and help facilitate conversations with family members or between patients and health care professionals.
